1. Introduction to risk assessment and management in major projects
A distinctive feature of the modern world is the need to instigate and successfully manage largescale mega projects which are designed to bring wider benefits to society at large. Such projects
may have technological, social, economic, legal and political aspects, but all imply the investment
and coordination of significant valuable resources, often with an international/global dimension, for
a prolonged time period to achieve specific long-term objectives.
Examples of large-scale mega projects include:
Construction of major transport systems (examples of HS2 and Crossrail)
Development in the automotive and aerospace industries of new designs of cars & airliners.
Building of offshore oil platforms and pipelines
Development of new drugs (example of search for Covid-19 vaccine)
Implementation of national public healthcare awareness programmes
Staging of major sporting events (Olympic Games and FIFA World Cup)
Development of global IT and database systems
Coordinating an international famine and refugee relief programme
Inter-governmental collaboration of fiscal policies to stabilise the global banking system
However, as the complexity and resources invested in projects grows, so does the associated risks
both in human terms and/or financial/business terms if a project completely fails or falls significantly
short of achieving its objectives. In a global survey by the Project Management Institute it was found
that, globally, organisations wasted an average of $122 million per $1 billion invested in 2016 and
across all industries, the average percentage of projects that are deemed failures is 14%.
Well known examples of complete project failure or significant underachievement include:
Loss of the NASA space shuttles Challenger and Columbia
Rabbit Telecom and Mercury Communications.
General Motors EV1 electric car and Sinclair C5
Several commercial airliner designs: Concorde, Airbus A380 and Boeing 737-Max
United Nations response to genocide in Rwanda
Financial strategies of the Royal Bank of Scotland and Lehman Brothers
Apple Lisa and IBM Junior PC
Thalidomide
New taste Coca-Cola and McDonalds Arch Deluxe burger
Sony Betamax and Minidisc

A key aspect identified in the subsequent investigations into all of the above project failures was that
the risk assessment and management processes associated with them were either completely
absent, poorly conducted or the results ignored. In all cases, the true risks were found to be
significantly higher than was originally envisaged at the outset of the projects.

2. Aim of individual coursework
The aim of the coursework is to prepare a report which selects and critically analyses a major project
which either failed completely, or significantly underachieved in respect of its risk assessment and
management aspects. In addition, the report will identify the key lessons and insights gained from
the failures and make recommendations for the future.
Important please note that the supplementary assessment is researched, written and submitted as
an individual piece of coursework. It will similarly be marked and graded on that basis.
3. Choosing a suitable project failure case study to write about.
In seeking a identify a major project failure or underachievement to write an individual report about,
you need to be proactive and to first investigate it to see if you can obtain enough information to
build up into a detailed case study. Do not underestimate this task, as while you may be able to
quickly think of a large-scale project where a major failure has occurred, you need to remember that
the main aim of the coursework is to critically analyse the risk assessment and management aspects
of it and finding detailed information on this may not be readily available. Some project failures have
a lot more information published about their risk assessment and management processes than
others and it takes time to identity this.
Important – When you have identified an appropriate project failure to use as a cast study, please

is suitable to use.
4. Structure of report
Your individual report needs to critically analyse and discuss the following points and will be
assessed by the following weightings:

For what purpose(s) was was the project instigated and its scope?
Outline the intended timescale of the development and implementation of
the project. Identity the key milestones where appropriate.
2. Project stakeholders
Who were the key stakeholders associated with the project and what were
their individual objectives, requirements and conflicts of interest?
What were the time cost quality constraints facing the project when its
feasibility by the stakeholders was evaluated?
How was the leadership of the project and its management organised?

3. Insights into risk assessment gained from previous failed projects
Did the stakeholders involved have previous experience of assessing and
managing risks in this type of project?
Was there information available from other successful and less
successful/failed projects, in both similar and/or related contexts, that could
have been used to drawn upon to properly assess the risks involved?
Outline these with relevant examples.
4. Risk assessment and management failures
Describe at least five major failures of the project and explain their causes.
For each failure, refer critically to what risk assessment tools and risk
management methods could have been implemented to either mitigate the
failure of the project or reduce its escalation.

Present your conclusions as a discussion of the insights gained from the
failed or underachievement of the major project studied in respect of risk
assessment and management. Include also, your recommendations for the
future risk evaluation of other similar mega projects.

7. Assessment and submission details
The supplementary assessment of the module is assessed via an individual report as per the
specification outlined and weighted at 100%.
The length of the coursework is 4000 words, excluding the table of contents, lists of figures,
diagrams, & tables and lists of references and appendices.
The major project failure you select for the individual coursework case study cannot relate or
feature the same organisation/company that you used for the previous group coursework.
Your individual case study must be based on completely new material.
Your report should be submitted via Canvas as Microsoft Word file. Please note that all
other file formats and zipped folders are not acceptable.
The submission deadline for the coursework is 18.00 hours on Friday 31st July 2020
